Position Summary:
Coordinate and manage care for members with complex physical health needs, creating personalized care plans and supporting high-quality, cost-effective outcomes for members and their families.
Must reside in Michigan for member home visits within Wayne and Macomb counties.
Travel Requirement:
Travel up to 50% required
Mileage reimbursement for costs incurred during field visits
Education and Experience:
Active Michigan RN license or Compact State Licensure required
or
Master's degree in Behavioral Health or Social Work
2 – 4 years of related experience.
Pay Range:
$56,200.00 - $101,000.00 per year
